Death and Hades were thrown into the lake of fire. This is the second death, the lake of fire. If anyone was not found written in the book of life, he was cast into the lake of fire.

A fiery landscape of orange and yellow flames on a canyon, with large jagged cliffs emerging from either side. The cliff tops are covered with a vast crowd of people.
Above the hellish terrain, billowy clouds rise casting a shadow onto the landscape. Within this cluster of clouds are immense lightning flashes.
Above them, a dark blue sky full of stars and gaseous nebulae provide the impression of a cosmic event, with bolt of light falling from heaven to earth.
Overall a scene of final judgement.
